What is Social Prescribing?
Social Prescribing is a service ran by local NHS services, charities, organisations and GP surgeries, through a Link Worker. Link workers work in the community with clients and local services, to offer practical and emotional support.
Our Community Fund Pot.

We work collaboratively with charities, organisations and Link workers across Middlesbrough and Stockton. We offer spaces to those who need it across Teesside with our ‘Pay It Forward‘ scheme and support from The National Lottery Community Fund, Middlesbrough Council & The Allen Lane Foundation.

What can we help with?
We help support a wide range of people across Teesside, including people:
- with one or more long-term conditions (Such as but not limited to COPD, Early on-set Dementia, Parkinsons and Multiple Sclerocis)
- with joint and mobility problems
- who need support with mild-moderate low mood and anxiety
- who are isolated and lonely
- who want to be proactive about looking after their physical and mental-wellbeing

If you are interested in social prescribing and how link workers can support you, please speak to your local GP surgery or contact local mental health charities and organisations.
