What is Social Prescribing?

Social Prescribing is a service ran by local NHS services, charities, organisations and GP surgeries, through a Link Worker. Link workers work in the community with clients and local services, to offer practical and emotional support.

We work collaboratively with Link workers across Middlesbrough and Stockton, to offer our services.

What can we help with?

We help support a wide range of people across Teesside, including people:

  • with one or more long-term conditions (Such as but not limited to COPD, Early on-set Dementia, Parkinsons and Multiple Sclerocis)
  • with joint and mobility problems
  • who need support with mild-moderate low mood and anxiety
  • who are isolated and lonely
  • who want to be proactive about looking after their physical and mental-wellbeing

If you are interested in social prescribing and how link workers can support you, please speak to your local GP surgery or contact Middlesbrough and Stockton Mind.
